Thank you for your confidence in ASCIP by allowing us to continue to meet your risk management and insurance needs.

To better serve your renewal process, we have launched our new online system, which we trust you will find user-friendly, especially when most of us are currently working remotely.

The information provided will be used to determine your premium for the Property & Liability and Workers’ Compensation programs for the upcoming year.

The questionnaire is divided into four sections:

1. Section 1: Property and Liability Program Premium Exposure Information – due by June 30, 2021
2. Section 2: Risk Management Exposure Information – due by June 30, 2021
3. Section 3: Cyber & Ransom Questionnaire (PDF attached separately) – due by June 30, 2021
4. Section 4: Contacts (PDF attached separately) – due by June 30, 2021

These four sections may be worked on independently, but only one person should work on a given section at a time.

If you are unable to complete this questionnaire in a single session, use the green “Save” buttons to save your progress, and you can return to the questionnaire at a later time using the passcode you created.
